Cedar Ridge Animal Hospital offers our pet patient form(s) online so you can complete it in the convenience of your own home or office.  Call 816-833-1300 with any questions.

This let’s us record our pet and owners into our management software so that we can effectively and efficiently provide the service you deserve!

Pet Treatment Consent

If you are dropping off your pet for the day, whether for an annual exam or hospitalization, we require a Treatment Consent form to be signed stating you agree we can treat your pet for items specified by you.

Sick Pet Release - Required

If you are dropping your pet off for us to examine him while he is sick, please fill out the following form so that we can have a better understanding of how he is feeling.

Pet Surgery Forms - Required

If you are dropping your pet off for surgery we require that one of the following forms be filled out, depending on what type of surgery he/she will be having. All surgeries performed at Cedar Ridge Animal Hospital are required to have Pre-Anesthetic blood work done before any pet will be sedated. We also require an IV Catheter and Fluids for all surgeries, except feline neuters. We offer other procedures which are optional to you, yet highly recommended. Elective Surgery - This is a surgery that you choose to have done on your pet (i.e. Spay, Neuter, Dental Cleaning, etc.). Click Here to View and Print

Non-Elective Surgery - This is a surgery that is done out of the well being of your pet (i.e. Knee Repairs, Bladder Stone Removals, etc.). Click Here to View and Print

Feline Neuter - Because it takes such little time to perform this surgery, we do not require they are placed with an IV Catheter and Fluids. It is still an option you can choose to have performed. Click Here to View and Print

Feline Declaw - This form is for those that are just getting declawed. Click Here to View and Print

Feline Declaw with a Spay or Neuter - This form is for those that are being declawed along with a spay or neuter. Click Here to View and Print

Member Wellness Registration Form - Optional

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office.
